The BHN Model of Care

The BHN Model of Care was developed through a comprehensive co-design process with clients, families, carers, consumer and lived / living experience representatives, volunteers, and staff.


The BHN Model of Care provides high-level guidance and direction to inform the design, delivery and continuous improvement of our services and programs, and our approach to community development, advocacy, and collective action. It describes how BHN will organise and deliver care, particularly the client pathways to and through our services.


The Functional Attributes of the Model of Care:

  • Makes community health and disability services and programs an exceptionally positive and effective experience for clients and staff
  • Ensure provider partners find working with us enjoyable, effective and efficient
  • Places BHN at the forefront of community health and disability service delivery innovation

 

In reviewing the Model of Care, we will always ask, “will this change make it better for clients, community and staff?”
